Not to display (again) my ridiculous naivete, but what exactly is a half and half?

It was on the Sopranos last week, and I can't get a consensus on what it means. Half oral sex, half manual?

Ah! Great! We had a long discussion about this after the show. Was she offering Junior a cup of coffee or what? Our best guess was oral sex with a hand job to complete the deed, but I'm very pleased to see a definitive answer. I will now share it with all my Sopranos-watching friends.
